Patrick’s Journey to Improved Wellbeing with Absolute Care Services
Background:
Patrick, a resident of his own home, faced severe anxiety, depression, and significant weight loss, leading to other health concerns. In November 2023, he transitioned to Absolute Care Services (ACS), seeking a supportive provider to help regain his health and wellbeing. Despite initial reservations, Patrick found the change to be beneficial, attributing his improved confidence and physical health to the care he received.
The Support Provided by ACS:
ACS implemented a comprehensive daily care package tailored to Patrick’s needs, consisting of three visits per day. The care plan was designed to promote his independence, ensure his physical and mental well-being, and integrate him into the community.
- Promoting Independence and Community Integration:
ACS’s care manager worked closely with Patrick to set achievable goals, focusing on maintaining independence and engaging with the community. Patrick was encouraged and supported to participate in social activities, helping him overcome feelings of loneliness and build new friendships. This included supporting him to establish and lead a health and wellbeing group, which now has four regular attendees discussing mental health topics.
- Enhancing Physical Health and Nutrition:
Recognising Patrick’s cultural preferences, the care team arranged for culturally appropriate meals, including Caribbean dishes, which significantly improved his appetite and contributed to his weight gain. This personalised approach helped Patrick reconnect with his cultural roots, enhancing his overall well-being.
- Mental Health Support:
The care team provided consistent emotional support, helping Patrick manage his anxiety and depression. By creating a supportive environment where he felt understood and valued, Patrick’s mental health improved, allowing him to engage more fully in community activities and build a supportive network.
Outcomes:
Patrick’s experience with ACS showcases the positive impact of tailored care and support. He reports feeling happier, more confident, and physically healthier. The initiative to lead a health and wellbeing group has not only helped Patrick but also benefited other residents, fostering a supportive community atmosphere.
According to Patrick, “Living in my own home has been really good for me. I feel much better now, even though I was hesitant at first. The care manager listened to what I wanted to achieve and supported me every step of the way.”
